Northwood users are able to request custom web forms that can be linked to the public site or within my.northwood.edu. These forms can be things like event sign-ups, employment applications, private donor forms and more. All requests for creation or modifications to forms.northwood.edu, nusites.northwood.edu or start.northwood.edu should be completed by using the Submit IT Request button.

After you have completed the request form, a ticket confirmation message will be sent to your Northwood email account. Your ticket will be placed in an IT queue where it will shortly be assigned a technician who will work with you to resolve your request.
